I like this a lot, I just wish it were a editable document. I do not have great writing and like to be able to hand everything over to the intervention specialist who has my students next year everything I have on them, including these. Is there anyway to get an editable document of it?

I love this! Would you be willing to add a row that states Mainstreamed Y/N Time?

This is a really great at a glance form! I teach students with mod/severe disabilities, so it is nice to see one that is not specifically for students on a resource caseload! Thank you so much! The one thing I would LOVE is if it was editable!

Wish it was editable, just so I could tweak a couple of things. But overall, I like it! I’m a first year SpEd teacher (self-contained!) so this template will be great for me. I downloaded this one specifically because it included a small section for parent info–I won’t have to keep a separate section for parent info. Thank you!
